Community Habilitation
The Community Habilitation Program services are designed to assist individuals in acquiring, retaining and improving the self-help, socialization and adaptive skills necessary to become as independent as deemed possible.
The Community Habilitation Program provides regularly scheduled one-on-one visits and supports to help an individual reach his/her valued outcomes and goals that have been identified in their Life Plan. This service is provided to the person in a community setting or when necessary in his/her home. Services are usually offered in two to three hour increments a few times per week, in special circumstances there may be additional hours provided.

Eligibility
This service is available to people who range in age from adolescence to senior citizens. He or she must have a diagnosis of an intellectual or developmental disability and be enrolled in Medicaid and be Waiver eligible.
